Ezi Practice Manager, LLC

These Terms of Service ("Terms") govern your access to and use of EziGoals, a clinical goal management platform operated by Ezi Practice Manager, LLC ("Company," "we," "our," "us"), a Georgia limited liability company located in Morrow, Georgia. By creating an account or using the platform, you agree to these Terms. If you are using EziGoals on behalf of a practice, clinic, or organization, you represent that you have the authority to bind that entity to these Terms.
EziGoals is a web-based clinical goal management platform designed for Board Certified Behavior Analysts (BCBAs) and Applied Behavior Analysis (ABA) practitioners. The platform provides assessment scoring, goal recommendation, program design, teaching instruction generation, and treatment plan documentation tools.
EziGoals is NOT an Electronic Health Record (EHR). It does not provide data collection, session notes, or billing/claims processing. EziGoals does not provide clinical advice, diagnoses, or treatment determinations. It is a clinical decision-support tool only.
All goal recommendations, clinical pathway suggestions, program parameters, and treatment plan language generated by EziGoals are decision-support outputs only. The treating Board Certified Behavior Analyst retains full clinical responsibility for all treatment decisions, goal selection, program modifications, and documentation accuracy.
EziGoals does not practice behavior analysis. The platform assists clinicians by organizing peer-reviewed research in Applied Behavior Analysis and established assessment frameworks into structured, clinically appropriate recommendations. Final clinical judgment always remains with the supervising BCBA.
The platform's recommendations are informed by published research including but not limited to works by Sundberg (2008, 2014), Partington (2006), Dixon (2014), Carr and Durand (1985), and other peer-reviewed sources in the ABA literature. These recommendations are not a substitute for individualized clinical assessment and professional judgment.
You must be at least 18 years old to create an account. You must provide accurate and complete information during registration, including your full name, email address, and practice name. You are responsible for maintaining the confidentiality of your login credentials and for all activity that occurs under your account.
Each user must have their own individual login. Sharing account credentials is strictly prohibited and may constitute a violation of HIPAA requirements. If you become aware of any unauthorized use of your account, you must notify us immediately at support@ezigoals.com.
The account owner (the person who created the practice account) is responsible for managing team members, assigning roles, and ensuring all users within their practice comply with these Terms.
EziGoals processes Protected Health Information (PHI) as defined by the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and the Health Information Technology for Economic and Clinical Health Act (HITECH Act).
We implement the following safeguards to protect PHI:
Technical Safeguards: All data is encrypted in transit using TLS 1.2 or higher and encrypted at rest using AES-256 encryption. Row-level security (RLS) policies enforce data isolation at the database level. Role-based access controls restrict PHI access to authorized users only. All authentication is handled through secure session management with automatic expiration.
Administrative Safeguards: Access to production systems is restricted to authorized personnel. Security incident response procedures are maintained. Platform access is logged for auditing purposes.
Multi-Tenant Data Isolation: Each practice's data is logically separated at the database level using tenant-scoped queries and row-level security policies. No practice can access, view, or modify another practice's data under any circumstance. All database queries are scoped by tenant identifier.
Covered entities (ABA practices that transmit health information electronically) using EziGoals are required to execute a Business Associate Agreement (BAA) with Ezi Practice Manager, LLC prior to entering any Protected Health Information into the platform. To request a BAA, contact support@ezigoals.com.
We will notify affected account owners of any confirmed data breach involving PHI within 60 days of discovery, in accordance with the HIPAA Breach Notification Rule.
You retain full ownership of all data you enter into EziGoals, including but not limited to client profiles, assessment scores, treatment goals, program parameters, teaching instructions, treatment plans, and any other content you create within the platform.
We do not sell, rent, trade, license, or otherwise share your data with any third party for marketing, advertising, research, or any purpose other than providing the EziGoals service.
We do not use your client data or Protected Health Information to train artificial intelligence models, improve algorithms, or develop new products. Your clinical data is used solely to provide the services you requested within the platform.
We may collect and analyze anonymized, aggregated usage data (such as feature usage patterns, page views, and performance metrics) that cannot be traced back to any individual client or practice. This data is used solely to improve the platform's performance and functionality.
EziGoals offers four subscription tiers: Starter ($15/month), Professional ($39/month), Clinic ($79/month), and Enterprise ($299/month). Annual billing is available at a discounted rate. Prices are listed on our website and may be changed with 30 days' written notice to existing subscribers.
New accounts begin with a 14-day free trial that includes full access to the platform. No credit card is required to start a trial. At the end of the trial period, you must select a paid plan to continue accessing your data and using the platform.
All payments are processed securely through Stripe, Inc. We do not store credit card numbers, CVV codes, or full payment details on our servers. Stripe's security practices are described at stripe.com/docs/security.
Subscriptions are billed monthly or annually in advance. Refunds are not provided for partial billing periods. You may cancel your subscription at any time through the Settings page. Upon cancellation, your access continues until the end of the current billing period.
Each subscription tier includes specific limits on the number of clients, users, assessment libraries, and automation features available. These limits are described on our pricing page and within the platform.
If you exceed the client or user limit for your tier, you will be prompted to upgrade. We will not delete your existing data if you exceed a limit, but you may not be able to add new clients or users until you upgrade or remove existing records.
Certain features, including the AI Assessment Score Reader, Vineland-3 library access, and Ezi Practice Manager integration, are available only on higher-tier plans as specified on our pricing page.
You agree to use EziGoals only for lawful purposes consistent with the practice of Applied Behavior Analysis. You agree not to:
Use the platform to store, transmit, or process data in violation of HIPAA or any applicable federal, state, or local law.
Attempt to access data belonging to other tenants or users outside your practice.
Share your login credentials with others or allow unauthorized individuals to access the platform through your account.
Reverse engineer, decompile, disassemble, or attempt to derive the source code of any part of the platform, including the clinical recommendation engine, goal graph, or assessment scoring algorithms.
Use automated scripts, bots, or other tools to scrape, extract, or copy data or content from the platform.
Upload malicious code, viruses, or any material that could compromise the security or integrity of the platform.
Reproduce, distribute, or commercially exploit the platform's goal libraries, clinical pathways, teaching instructions, or any other proprietary clinical content.
The EziGoals platform, including its software, design, clinical recommendation engine, goal graph, assessment scoring algorithms, teaching instruction generator, treatment plan writer, clinical pathway logic, and all original goal library content (including the EziGoals Skills Assessment), is the proprietary intellectual property of Ezi Practice Manager, LLC.
The goal libraries within EziGoals are organized from peer-reviewed research in Applied Behavior Analysis and established assessment frameworks. The specific implementation, structure, prerequisite chains, clinical pathway mapping, tier progression logic, and generated clinical language are original works created by Ezi Practice Manager, LLC.
You may not copy, reproduce, distribute, modify, create derivative works from, or commercially exploit any part of the platform or its content without prior written permission from Ezi Practice Manager, LLC.
You retain ownership of your data as described in Section 5. Your use of the platform does not grant you any ownership interest in the platform's software, algorithms, or clinical content.
EziGoals uses the following third-party services to operate the platform:
Supabase: Database hosting, authentication, and row-level security. Data is stored on infrastructure located in the United States. Supabase maintains SOC 2 Type II compliance.
Vercel: Application hosting and content delivery. Serves the web application globally with edge caching.
Stripe: Payment processing. Handles all billing information securely. Stripe is PCI DSS Level 1 certified.
Resend: Transactional email delivery for notifications (trial expiry alerts, mastery milestones, authorization reminders).
Anthropic: AI-powered assessment score extraction from uploaded scoring form images. Available on Enterprise tier only. Uploaded images are processed and not retained by the AI provider after processing.
Each third-party service processes data only as necessary to provide its respective function. We select third-party providers that maintain appropriate security certifications and data protection practices.
Active Accounts: Your data is retained for the duration of your active subscription.
Canceled Accounts: Upon cancellation, your data is preserved for 90 days. During this period, you may reactivate your subscription and regain access to all your data. After 90 days, your data may be permanently deleted from our systems.
Expired Trials: Trial account data is preserved for 90 days after the trial expires.
Notification Logs: Internal notification delivery records are retained for 12 months for operational purposes.
Payment Records: Billing records are retained as required by applicable tax and financial regulations.
You may request deletion of your account and all associated data at any time by contacting support@ezigoals.com. We will process deletion requests within 30 days, subject to any legal retention requirements.
You may export your data at any time using the Data Export feature in the Settings page. Exports are available in JSON (complete data) and CSV (goal summary) formats.
To the maximum extent permitted by applicable law, Ezi Practice Manager, LLC shall not be liable for any indirect, incidental, special, consequential, or punitive damages, including but not limited to loss of profits, data, business opportunities, or goodwill, arising out of or in connection with your use of or inability to use the platform.
Our total aggregate liability to you for all claims arising out of or relating to these Terms or your use of the platform shall not exceed the total amount paid by you to Ezi Practice Manager, LLC for the platform during the twelve (12) months immediately preceding the event giving rise to the claim.
EziGoals is a clinical decision-support tool. We are not responsible for clinical outcomes, treatment decisions, or documentation submitted to insurance companies or other third parties. The supervising BCBA is solely responsible for all clinical decisions made using information provided by the platform.
The platform is provided "as is" and "as available" without warranties of any kind, either express or implied, including but not limited to implied warranties of merchantability, fitness for a particular purpose, and non-infringement.
We do not warrant that the platform will be uninterrupted, error-free, or completely secure. We do not warrant that any clinical recommendations, goal suggestions, or treatment plan language generated by the platform will be appropriate for any specific client or clinical situation.
You agree to indemnify, defend, and hold harmless Ezi Practice Manager, LLC, its officers, directors, employees, and agents from and against any claims, liabilities, damages, losses, and expenses (including reasonable attorney fees) arising out of or in connection with: (a) your use of the platform; (b) your violation of these Terms; (c) your violation of any applicable law or regulation, including HIPAA; or (d) any clinical decisions made using information provided by the platform.
Either party may terminate these Terms at any time. You may cancel your subscription through the Settings page. We may suspend or terminate your account if you violate these Terms, fail to pay subscription fees, engage in conduct that threatens the security or integrity of the platform, or for any other reason with 30 days' written notice.
Upon termination, your data will be handled in accordance with Section 11 (Data Retention and Deletion).
We reserve the right to immediately suspend or terminate access without notice in cases involving: unauthorized access to other tenants' data, introduction of malicious code, violation of HIPAA, or any activity that poses an immediate security risk to the platform or its users.
These Terms are governed by and construed in accordance with the laws of the State of Georgia, United States, without regard to its conflict of law provisions. Any dispute arising from these Terms or your use of the platform shall be resolved in the state or federal courts located in Clayton County, Georgia.
Before initiating any legal proceeding, you agree to first attempt to resolve the dispute informally by contacting us at support@ezigoals.com. We will attempt to resolve the dispute through good-faith negotiation within 30 days.
We may update these Terms from time to time. When we make material changes, we will notify the account owner via email at least 30 days before the changes take effect. The updated Terms will also be posted at ezigoals.com/terms with the revised effective date.
Your continued use of the platform after the effective date of updated Terms constitutes your acceptance of the changes. If you do not agree to the updated Terms, you must stop using the platform and cancel your subscription before the effective date.
If any provision of these Terms is found to be unenforceable or invalid by a court of competent jurisdiction, that provision shall be enforced to the maximum extent permissible, and the remaining provisions shall remain in full force and effect.
These Terms, together with our Privacy Policy and any executed Business Associate Agreement, constitute the entire agreement between you and Ezi Practice Manager, LLC regarding your use of the EziGoals platform. These Terms supersede any prior agreements, communications, or understandings regarding the platform.
